The sale of insurance represents a contract between the insurance provider, who carries the risk your pet will need treatment and you, who is responsible for paying insurance premiums. This contract typically carries an expiration date of one year from the effective (start) date unless you choose to renew the policy.

The steps you need to take include: (1) complete and sign a claim form, (2) get veterinary medical records (SOAP notes) from the incident, and (3) get an itemized invoice from the incident. If your pet is less than 12 months of age, get all vet visit medical records. If your pet is older than 12 months (not adopted) pinclude the past 12 months of medical records at least. If you adopted our pet, include your adoption records.
You may file for reimbursement from your insurance provider for treatment and covered services that occurred within this number of days.
